Recovery Readiness Check

For anyone recovering from an injury, operation, stroke or new diagnosis: checks whether the practical, physical and emotional pieces of recovery are in place, and flags warning signs that need a doctor today or emergency care.

  2. I know which medicines to take, how, and for how long.

  3. My pain is controlled well enough to move, sleep and do my exercises.

  4. I have a follow-up appointment or know where to go for check-ups (clinic, hospital, physio).

  5. I am doing the exercises or activity my therapist or doctor gave me.

  6. I am afraid to move in case I hurt myself again.

  7. I have someone to help at home with meals, washing, transport or childcare.

  12. Is a wound red, hot, swelling, leaking pus or smelling, or do you have a fever or chills?

  13. Do you have new face drooping, arm or leg weakness, slurred speech, sudden confusion or a sudden severe headache?

  14. Have you had thoughts of harming yourself or that life is not worth living?
