Physical Activity Check

Gives a quick picture of how much you move, sit and strengthen your body compared with general adult guidance, and checks whether you should speak to a doctor before getting more active.

Think about a normal week in the last month. Walking to the taxi rank, carrying water, gardening and housework all count. Private. Your employer never sees your answers. This is a self-check, not a diagnosis, and you can stop anytime.

  1. On how many days a week do you do at least 30 minutes of activity that makes you breathe a bit faster (brisk walking, dancing, cycling, heavy housework)?

  2. On how many days a week do you do something that works your muscles (lifting, squats, carrying, push-ups, digging)?

  3. How many hours a day do you sit in total (work, taxi or car, TV, phone)?

  4. How often do you sit for more than an hour without getting up?

  5. Climbing a flight of stairs or walking uphill leaves me very out of breath.

  6. I feel too tired, sore or low to move.

  7. Safety, cost, time or no safe place to walk stops me from being active.

  8. I have had a fall or nearly fallen in the last 6 months.

  9. Do you get chest pain, tightness or pressure when you are active, or at rest?

  10. Do you lose balance because of dizziness, or have you fainted in the last year?

  11. Has a doctor told you that you have a heart condition or high blood pressure, or that you should only exercise under supervision?

  12. Are you pregnant, recovering from an operation or injury, or living with a condition such as diabetes, asthma or arthritis?
